This is a "Get Acquainted" appointment. This service is designed for anyone who needs to get established with a caring local provider for routine Primary Care. Ideal for patients with chronic care needs (ADD/ADHD, diabetes, high blood pressure, high cholesterol, thyroid problems, etc.) who are not acutely ill, but need to be medically managed. We will get to know you and create a medical management plan tailored to your specific needs. Service includes transfer and review of past medical records, complete physical exam, review and management of medications, and prescription refills as needed.
